Why do I need to provide my contact details?

We are asking for your contact details to enable us to be able to respond directly to your feedback to improve the services that you receive. Your feedback in valuable, and we would like to be able to respond directly to you if the need arises. The survey is designed to assist us in refining our services at a local level, therefore understanding the specific needs of the clients in the areas in particularly important to us.  

Who will see the feedback that I provide?

The Regional Practice Leader in your area will be the key person analysing the feedback that is received. Where appropriate, this feedback will be shared with your Service Provider and the Regional Manager. The Clinical Governance Team at Vision Australia will have oversight of all the feedback received from this survey and plans developed to respond to the feedback.
